Guardian Home Health Hospice is seeking a Medical Social Worker (MSW) to join our hospice team. The Medical Social Worker (MSW) will participate in the development, implementation and revision to the hospice patient's plan of care. This individual will ensure appropriate psychosocial services are provided to hospice patients and families/caregivers. As part of this team, the MSW will:
1. Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
2. Comply with applicable legal requirements, standards, policies and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPAA and Documentation Standards.
3. Demonstrate service excellence, guiding principles and mission of Texas Home Health Hospice in work performance.
Required Skills
1. Demonstrates good verbal and written communication skills.
2. Demonstrates good organizational skills.
Required Experience
1. Masters of Social Work Degree from a school of Social Work accredited by the Council on Social Work Education.
2. Minimum of one (1) year experience required in healthcare setting; hospice preferred.
3. Possesses and maintains current CPR certification.
4. Understands hospice philosophy, and issues of death/dying.
5. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
